We are here to help you and your pet
We know referral to a veterinary specialist can be a source of anxiety or concern in some cases. Please know we are here to aid your family veterinarian in achieving the highest quality compassionate care and advanced diagnostics for your pet. We believe good communication is important in providing optimal patient care. To that end, we request all family members with information relevant to their pet’s history, care and decisions be present for their initial appointment if at all possible.

During your first appointment
We want to spend the majority of your first appointment talking with you, examining your pet, evaluating the previous medical records and making recommendations for future diagnostics and therapy. An estimate of costs for these recommendations will be given as well. We encourage you to ask questions and share your concerns. In order to have time for this thorough approach, it is vital that you be on time for your appointment, along with bringing your pet’s medical records. New appointments should arrive 15-20 minutes prior to their scheduled appointment time so that all records can be completed prior to seeing the doctor.

  • Please complete all of the forms below and bring them with you to the office. If you complete online, these will be automatically submitted to the office.
  • Please arrive at least 20 minutes early so your paperwork can be processed.
  • Please bring a copy of all medical records and graphic data/images.
  • Please bring your pet’s medications if unable to complete the med list.
